Matriculating Students

All incoming students must complete and send in Health Records upon matriculation

Forms must be  completed and completed records must be uploaded to our secure patient portal.

FORMS AND DOCUMENTATION

ALL incoming students are REQUIRED to complete a health report once upon matriculation.

Students will need to have their health provider complete and sign the required ***health record documents***.

These forms (download here) include the required immunizations, tuberculosis screening requirements and a sample physical exam form.

Once your required health documents are prepared students can go to the Secure Patient Portal and:

1. Complete the 3 required on-line forms found under the Forms tab: Health History Form, Tuberculosis Screening Questionaire, and the General Agreement.

2. Then, under the Immunization tab enter the dates of your vaccines from your record.

3. Finally, from the Upload tab, upload your required medical documents for verification.

We will communicate any discrepancies or further needs through the secure patient portal by sending a link to your Brandeis email account.

Failure to complete these steps will result in a registration hold.

Medical Record Release/Request    Use this form to request copies of your medical records which are kept at Brandeis Health Center for 10 years.

Meningococcal Waiver Form -For students living on campus to provide informed declination of required Meningococcal vaccine (this is not recommended).
