The following printable documents for use by members of the Brandeis community involved in the student conduct system must be printed, completed and signed, then returned to the Department of Student Development and Conduct. Forms may not be submitted electronically. Paper copies of all forms are available from the Department of Student Development and Conduct in Shapiro Campus Center, Mailstop 206, (781) 736-5070.
- A completed Incident Report/Referral Form or Academic Integrity Referral Form, depending upon the nature of the allegation, initiates the conduct process. Additional documentation, such as a copy of an academic assignment, an incident report, or a personal statement, may be attached to the referral.
- The accused student completes a Choice of Action Form, which indicates his or her acceptance or denial of responsibility. This form must be signed and dated by the student, and returned to the designated administrator within 48 hours of receipt of the written charges.
- After the completion of a conduct matter, a sanctioned student may choose to submit an Appeal Hearing Request Form, if the student was treated improperly. The University Appeals Board on Student Conduct will meet to review the appeal request.
- The Waiver of Procedural Rights Form itemizes the rights students have under the Brandeis Student Conduct System, and allows a student to waive one or more of these procedural rights in order to expedite the conduct process.
