Forms/Survey/LaTex Program

Home / Students / Dissertation Guide / Forms/Survey/LaTex Program
  • Dissertation Examining Committee Form (PDF) — due two weeks prior to the defense date; if possible, you should have already filed your degree application online

  • Survey of Earned Doctorates (required for degree completion). The survey may be submitted via hard copy PDF or completed online: SED (PDF submission) or SED (electronic submission)

  • GSAS PhD Exit Survey (an invitation to complete the survey will be sent to your Brandeis e-mail account)
