Reason for Nomination
How has the nominee's mentoring enhanced your graduate education and professional and personal development? Please submit a brief statement (500 word limit) of nomination explaining your relationship with your faculty nominee and why he/she has distinguished him/herself as a mentor for you. You may wish to consult the awards criteria for possible qualities which you may want to cite. Share specific stories and anecdotes as appropriate.
In addition, you may solicit up to two additional letters of support for your nominee from other students (optional) or alumni and attach them to this form.
All nominations will be kept strictly confidential.
