DEI Program and Service Request Form
Dear Colleagues,
New, progressive diversity, equity, and inclusion (DEI) efforts are underway at Brandeis, and the Division of Diversity, Equity, and Inclusion (DDEI) is eager to support learning and educational opportunities for everyone. In order to best meet your needs, please complete our Program and Service Request Form, which will give us some preliminary information about what you are requesting. We will follow up with you once we have processed your request; all service requests are considered underway only upon confirmation of receipt from the Office of Diversity, Equity, and Inclusion (ODEI) Education and Learning Initiatives.
Program and Service Request Form
Please submit one form per type of service requested. For example, if you are interested in professional development for your department as well as guidance on your proposal for a grant, fill out one form for the workshop you would like, and make a separate submission for the consultation. For workshops and professional development, please give at least two weeks’ notice, in order to allow for adequate time to prepare and deliver high-quality programming; otherwise, you will be asked to provide alternative dates.
