Health Insurance Reporting and Selection

To report (waive) or select coverage for the upcoming 2007-08 academic year, students may complete the Health Insurance Reporting and Selection process by completing either the

• Undergraduate Health Insurance Reporting/Selection Form, or the
• Graduate Health Insurance Reporting/Selection Form.

This form may be faxed to the Health Center at 781-736-3675. In accordance with Massachusetts law, students are required to either purchase insurance or to certify that they have comparable coverage each academic year.
