Faculty Leave

Faculty requesting academic leave, as outlined in the Faculty Handbook, should complete the Faculty Academic Leave Request Form (see instructions below*). Faculty academic leave types are outlined on pages 5 and 6 of the form. Requests for leaves under the Family and Medical Leave Act (FMLA) should be directed to the Office of Human Resources.

Instructions for Completing the Faculty Academic Leave Request Form

  • Faculty requesting an academic leave must download and complete pages 1 and 2 of the Faculty Academic Leave Request Form and submit the saved form to their Department/Program Chair by the School's internally set deadline, usually sometime in the fall.
  • The Department/Program Chair will review each applicant’s submission, complete page 3 of the form, and forward the form to the Academic Dean for consideration.
  • The Academic Dean will review each request and submit their recommendation on page 4 of the form to the Provost no later than December 15.
  • The Provost will review each request and indicate their decision on page 4 of the form. Faculty Affairs (Provost's Office) will notify the Academic Dean of the Provost's decision by no later than February 15.
