2026-2027 Missing Information Glossary
When processing applications for financial aid, our office will occasionally need additional documentation to verify or clarify the information we have. Below is a list of the documents we may request along with an explanation on what it is and where you can get that document.
Many of these forms will only require information from one person, and the document notification you receive will be marked as Student, Custodial Parent, and Non-Custodial Parent accordingly. If you’re unsure which parent is which, please check the federal guidelines.
Please note that you only need to submit the requested information. The list below covers all forms, not just those that you will need to complete.
General Financial Aid Application Requirements
CSS Profile: This form is required for students and custodial parents to complete if the student is applying for institutional funding. This can be done through College Board.
Noncustodial Profile: This form is required for non custodial parents to complete if the student is applying for institutional funding. This can be done through College Board.
FAFSA:This form is required for students and custodial parents to complete if the student is applying for federal funding. This can be done through the FAFSA.
FAFSA Consent for FTI: In order for the FAFSA to be processed, both the parent and student must give consent for Federal Tax Information, which means that the FAFSA will be able to pull your tax information directly from the IRS. Even if you did not file taxes, you must give consent when completing the FAFSA. Log in to StudentAid.gov, select FAFSA form from Dashboard, and select “Make a Correction” to provide consent and approval. This is required even if the parent/student did not file a U.S. federal tax return or any tax return at all.
FAFSA Electronic Signature: In order for the FAFSA to be processed, both the parent and student must sign it, which will be asked for at the end of the FAFSA. Student/parent must log in to StudentAid.gov, select the FAFSA form from the Dashboard, and select “Make a Correction” to provide the signature electronically.
Confirmation of Unsubsidized Only Eligibility Form: On the FAFSA, you answered “yes” to question 8, indicating your parents are unwilling to provide their information, limiting your aid eligibility to federal unsubsidized loan only. If you intended to answer the question this way, please complete a Confirmation of Unsubsidized Only Eligibility Form, which can be completed here. If this response was in error, please log back into the FAFSA and make a correction to add your parent(s) information.
Provisional Independent Status Clarification: On the FAFSA, you indicated you have unusual circumstances. As a result, you were granted provisional independent status on the FAFSA and were not asked to provide parent information. If you made a mistake, please log in to StudentAid.gov and add your parent(s) to the FAFSA. If you intended to answer the question this way, you must now clarify and document your unusual circumstance for a final determination of your dependency status for federal financial aid by completing the Provisional Independent Status Clarification Form.
Tax/Income Documents
(If you are unsure which parts of your tax return to submit, you may submit the entirety of it)
2024 Business Return: This requirement can be met through multiple different forms. We can accept the S-Corp, Partnership Return, Schedule C, or Schedule E.
2024 Partnership K-1: This document is a form issued annually for an investment in a partnership. The purpose of Schedule K-1 is to report each partner's share of the partnership's earnings, losses, deductions, and credits. This will be sent to you by the corporation you are a partner of.
2024 Partnership Return: This return is where you would report your income, gains, losses, deductions, credits, etc. associated with a partnership. This would be found in a copy of your tax return and can be requested from the IRS if it has been lost.
2024 S-Corp K-1: This document is a report of your share of a corporation's income, deductions, credits, and other items. This will be sent to you by the corporation you own a portion of.
2024 S-Corp: This return is where you would report your income, gains, losses, deductions, credits, etc. associated with the S corporation. This would be found in a copy of your tax return and can be requested from the IRS if it has been lost.
2024 Tax Return, 1040: This will be the first two pages of a tax return. If you do not have your return but you filed online, you can request a copy from whomever you filed through. Please note that the page 2 signature line must be signed by hand or through Docusign.
2024 Tax Return, 1040 Schedule 1: This document is used to report types of income that aren't listed on the 1040, such as capital gains, alimony, unemployment payments, and gambling winnings. This would be found in a copy of your tax return and can be requested from the IRS if it has been lost.
2024 Tax Return, 1040 Schedule 2: This document is used to report taxes not included in the basic tax forms. This would be found in a copy of your tax return and can be requested from the IRS if it has been lost.
2024 Tax Return, 1040 Schedule 3: This document is used for nonrefundable credits. This would be found in a copy of your tax return and can be requested from the IRS if it has been lost. If this form is being requested but you did not file a schedule 3, please send our office an email at sfs@brandeis.edu that you did not file that schedule.
2024 Tax Return, 1040 Schedule C:This document is used to report income or (loss) from a business you operated or a profession you practiced as a sole proprietor. This would be found in a copy of your tax return and can be requested from the IRS if it has been lost.
2024 Tax Return, 1040 Schedule D: This document is used to report the sale or exchange of a capital asset not reported on another form or schedule.This would be found in a copy of your tax return and can be requested from the IRS if it has been lost.
2024 Tax Return, 1040 Schedule E: This document is used to report income or loss from rental real estate, royalties, partnerships, S corporations, estates, trusts, and residual interests in REMICs. This would be found in a copy of your tax return and can be requested from the IRS if it has been lost.
2024 Tax Return Transcript: If a tax return cannot be found or if there is some error in it, the Tax Return Transcript can be requested from the IRS as a substitute.
2024 W-2: For any job worked, the employee will receive a W-2 at the beginning of the next calendar year. If you do not have your W-2 you can request a new copy from the employer.
2024 Wage and Income Transcript: If a W-2 cannot be accessed or was never received, the Wage and Income Transcript can be requested from the IRS as a substitute.
Asset Clarification Form: This form allows us to verify the assets a parent/student has if there was some sort of discrepancy or missing information in the information we received. This form can be found on our Website.
Documentation of Social Security Benefits: Recipients of Social Security benefits should have a breakdown of what they will be receiving. If you do not have this information, you can access it through the Social Security Association.
IRA/Pension Rollover Verification: This form verifies if any IRA/Pension distributions were rollovers or not and how much they were for. This form can be found on our Website.
Non-US Asset Documentation: This requirement can be resolved by submitting bank statements or any other official documentation of assets that you have outside of the USA.
Non-US Income Verification: This requirement can be resolved by submitting foreign tax returns or any other official documentation of income that you made outside of the USA for the given year.
Tax Non-Filing Statement: This form is for those that didn’t file taxes in the given year. If more than one family member didn’t file, we will need a separate form for each member. This form can be found on our Website.
Parent’s Business/Farm Supplement: This form is for those that own a company or farm outside of the USA and will ask for a breakdown of the farm/business’ income and expenses. This form can be found on our Website.
Appeal/Waiver Documents
2025 Tax/Income Information: See the Tax/Income Documentation section for a breakdown of each potential document.
Documentation of Year to Date Income: This requirement is typically met through pay stubs for the current year to show what your most recent income has been. However, we can accept any other official documentation that shows that information as well.
Non-Custodial Parent Waiver Petition: This form is the worksheet associated with waiving the requirement for Non-Custodial Parent information. This form can be found on our Website.
Non-Custodial Parent Waiver Student Statement: This requirement is the student’s explanation of the circumstances that have led to needing to waive the requirement for Non-Custodial Parent information.
Non-Custodial Parent Waiver Third Party Letter: This requirement is an official’s explanation and verification of the circumstances that have led to needing to waive the requirement for Non-Custodial Parent information. Possible options for an official third party are, teachers, guidance counselors, therapists, police officers, etc.
Projected 2025 Income Worksheet: This form gives a general estimate of the income that will be made for the current year. Please be aware that this estimate will be compared to your actual earnings at the end of the calendar year. This form can be found on our Website.
Laptop Grant: This form is for high-need students who are requesting financial assistance in purchasing a laptop for school purposes. These can be used for students who are looking to purchase a laptop or already have. The form can be found here.
Miscellaneous Documentation
Citizenship Status Documentation: For this requirement the student will need to submit some form of citizenship verification, such as a passport, birth certificate, or social security card. This can be done by bringing it directly to our office, or by submitting a copy electronically.
Citizenship- Original Presented In Person: For this requirement the student will need to bring some form of citizenship verification, such as a passport, birth certificate, or social security card, into our office to be verified.
Clarification of Marital Status: This is typically required if there has been a change in marital status or some sort of discrepancy in paperwork. A marriage or divorce certificate will resolve this.
Clarification of Sibling in College Form: This form is for students that have siblings enrolled in college at another institution and allows us to estimate how that will impact the family’s ability to pay for college expenses. This form can be found on our Website.
Entrance Counseling: This form is required for students who are taking out Stafford Loans, also known as Subsidized and Unsubsidized Loans. This form verifies that the student understands the commitment and responsibilities associated with loaning money. This form can be found through Federal Student Aid (FSA)
Documentation of Child Support Paid: This form will be filled out with a breakdown of the child support that you paid out in 2024. This form can be found on our Website.
Documentation of Child Support Received: This form will be filled out with a breakdown of the child support that you have received in 2024. This form can be found on our Website.
Independent Status Documentation: This is to be completed by students that have begun the process of becoming an independent student and is resolved by verifying that you have been living independently of your parents. It can change based on the situation, so we recommend that you contact our office if you are unsure of what to submit.
Identity Verification: This form is used to verify your identity with some form of photo identification, such as a driver’s license or passport. You can either complete this by coming into the office or by Zoom appointment.
Master Promissory Note (MPN): This form is required for students who are taking out Stafford Loans, also known as Subsidized and Unsubsidized Loans. This form functions as a loan agreement for these federal loans. This form can be found through Federal Student Aid (FSA)
Laptop Grant: This form is for students who already have received permission from their FA to get the grant. The student submits the form and then sends us a screenshot of the receipt. This can be found online Application
