Required From the Hiring Department

• Actual Wage Form (see fillable sample - pdf)

• I-129 Export Control Compliance Certification (see fillable sample - pdf)

Required Fees

Departments may request checks through Brandeis Accounts Payable. Please see more guidelines on the School of Arts and Sciences website.

• Filing Fee: $460
• H-1B Anti-Fraud Fee: $500 (Fee not required for H-1B visa extensions)
• Premium Processing Fee: $2,500. (Please contact us to understand if this petition should be submitted with the premium processing service. You may contact us at scholars@brandeis.edu.
• All checks should be made payable to "Department of Homeland Security." The address on the checks should be:
